Job summary

Optum is seeking a Registered Nurse Case Manager for Deaconess Hospice to deliver comprehensive hospice care and coordinate plans of care for patients in Mississippi. The role emphasizes nursing evaluation, collaborative care planning, and patient/family education to promote physical, psychosocial, and spiritual well-being within a compassionate home-care setting.

Responsibilities include initiating the plan of care, documenting progress, and collaborating with the IDG and physicians.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.

Responsibilities

• Makes the initial nursing evaluation to determine hospice eligibility within 48 hours of referral.
• Identifies patient/family needs and re-assesses at least every 15 days.
• Initiates and coordinates the plan of care with the IDG and attending physician.
• Documents problems, goals, interventions, and patient/family response to care.
• Instructs and supervises patient/family in self-care techniques when appropriate.
