Job summary

UnitedHealth Group in Biloxi, MS seeks a Registered Nurse Case Manager to oversee the Plan of Care for hospice patients, collaborating with the IDG and other professionals to support physical, psychosocial, and spiritual well-being.

You’ll perform initial nursing evaluations within 48 hours of referral, develop and implement care plans, document progress, and guide patients and families through the hospice journey with compassionate, evidence-based care.

Qualifications

• Current, unrestricted RN licensure in the state of practice.
• Current CPR certification or ability to obtain within 90 days of hire.
• Valid driver's license, vehicle insurance, and access to dependable transportation.

Responsibilities

• Makes initial nursing evaluation within 48 hours of referral to determine eligibility for hospice services.
• Identifies physical, psychosocial, and environmental needs and re-assesses at least every 15 days.
• Initiates and coordinates the plan of care; documents problems, goals, interventions, and responses.
• Collaborates with patient/family, attending physician, and IDG to provide daily care.
• Instructs and supervises patient/family in self-care techniques when appropriate.
