Job summary

UnitedHealth Group’s Deaconess Hospice in Natchez, MS is seeking a Registered Nurse Case Manager to oversee the Plan of Care for hospice patients, coordinating with the IDG and treating teams. You will evaluate, develop, and implement treatment plans based on nursing theory to promote physical, psychosocial, emotional, and spiritual well‑being for patients and families facing end of life.

Qualifications

Current and unrestricted RN licensure in the state of practice.
Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.
State MS requirements: 1+ years of RN experience; LPN substitution rules may apply.

Responsibilities

Makes initial nursing evaluation to determine hospice eligibility within 48 hours of referral.
Identifies physical, psychosocial, and environmental needs; reassesses at least every 15 days.
Initiates and coordinates the plan of care; documents progress and outcomes.
Collaborates with patient/family, physician, and IDG to provide daily care.
Instructs and supervises patient/family in self-care techniques when appropriate.
Maintains accurate clinical notes and adheres to care standards.
