Job summary

Optum is hiring a Registered Nurse Case Manager for Heart of Hospice in Mississippi. This role delivers the Plan of Care for hospice patients, collaborates with the IDG, and guides families through medical and emotional support in alignment with scientific nursing theory.

Responsibilities include initial nursing evaluation within 48 hours of referral, ongoing reassessment every 15 days, care coordination, documentation, patient/family instruction, and maintaining accurate clinical notes.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle or public transportation.
• CPR certification preferred or ability to obtain within 90 days.

Responsibilities

• Makes initial nursing evaluation to determine eligibility for hospice services within 48 hours of referral.
• Identifies physical, psychosocial, and environmental needs and re-assesses at least every 15 days.
• Initiates and coordinates the plan of care with the IDG.
• Documents problems, goals, interventions and patient/family responses.
• Collaborates with patient/family and attending physician daily.
• Instructs and supervises patient/family in self-care techniques when appropriate.
• Maintains accurate clinical notes on patient condition.
