Job summary

Heart of Hospice, a part of LHC Group and a member of the Optum family, is seeking a Registered Nurse Case Manager to oversee the Plan of Care for hospice patients in Mississippi. You will evaluate performance, collaborate with the IDG, and implement treatment strategies to support physical, psychosocial, emotional, and spiritual well-being of patients and families.

Salary for this role ranges from $67,660 to $101,490 annually for full-time employment, with a comprehensive benefits package,

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Valid driver's license with access to dependable vehicle or public transport.
• CPR certification preferred.

Responsibilities

• Performs initial nursing evaluation to determine hospice eligibility within 48 hours of referral.
• Identifies patient/family needs and re-assesses at least every 15 days.
• Initiates and coordinates the plan of care with the IDG.
• Documents problems, goals, interventions and patient/family responses to care.
