Job summary

UnitedHealth Group’s Deaconess Hospice in Mississippi seeks a Registered Nurse Case Manager to deliver the Plan of Care for hospice patients and collaborate with a multidisciplinary team. Duties include initial assessments, care planning, and ongoing documentation to ensure compassionate, high-quality end-of-life care.

The role emphasizes evaluating nursing performance, coordinating strategies based on nursing theory, and supporting patients and families facing death in diverse home settings.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current driver’s license, valid vehicle insurance, and access to a dependable vehicle or public transportation.

Responsibilities

• Makes the initial nursing evaluation to determine eligibility for hospice services within 48 hours of referral.
• Identifies patient/family physical, psychosocial, and environmental needs and re-assesses at least every 15 days.
• Initiates and coordinates the plan of care; documents problems, goals and interventions.
• Collaborates with patient/family, attending physician and IDG members to deliver daily care.
• Instructs and supervises patient/family in self-care techniques when appropriate.
• Maintains accurate clinical notes; functions in diverse home settings regardless of age, race, creed, or disability.
