Job summary

Deaconess Hospice, part of the LHC Group and Optum family, seeks a Registered Nurse Case Manager to lead the Plan of Care for hospice patients. You will evaluate performance, implement strategies, and collaborate with a multidisciplinary team to address physical, psychosocial, emotional, and spiritual needs in end-of-life care.

Requirements include an active RN license in Mississippi, a valid driver's license, and eligibility to practice in hospice settings.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Valid driver's license and access to reliable transportation.
• Ability to work in home hospice settings with diverse patient needs.

Responsibilities

• Perform initial nursing evaluations to determine hospice eligibility within 48 hours of referral.
• Develop and implement Plan of Care in collaboration with the care team.
• Coordinate and document patient care, goals, and outcomes.
• Educate patients and families on self-care and care plans.
• Collaborate with physicians and the interdisciplinary group to deliver daily care.
