Job summary

UnitedHealth Group, through Deaconess Hospice, part of the LHC Group, seeks a Registered Nurse Case Manager to oversee hospice patients' Plan of Care and promote physical, psychosocial, emotional, and spiritual well‑being.

This role requires swift initial nursing evaluation, ongoing assessment, plan initiation and coordination, thorough documentation, collaboration with the patient’s family and attending physician, and education of families in self‑care techniques.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current CPR certification or ability to complete within 90 days of hire
• Current driver’s license, valid vehicle insurance, and access to a dependable vehicle, or public transportation
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client

Responsibilities

• Makes the initial nursing evaluation for hospice eligibility within 48 hours of referral.
• Identifies physical, psychosocial, and environmental needs and reassesses at least every 15 days.
• Initiates and coordinates the plan of care.
• Documents problems, goals, interventions, and patient/family response to hospice care.
• Collaborates with patient/family, attending physician and IDG members in daily care.
• Instructs and supervises patient/family in self-care techniques when appropriate.
• Maintains accurate clinical notes on patient condition.
