Job summary

Heart of Hospice is recruiting an RN Case Manager to oversee the Plan of Care for hospice patients in Georgia. You will evaluate and adjust care plans, collaborate with a multidisciplinary team, and promote physical, psychosocial, and spiritual well-being in end-of-life care.

The role requires active RN licensure in the state, a valid driver’s license, and CPR certification (or obtainable within 90 days). Prior clinical experience is valued. Benefits and a supportive team culture are emphasized.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Valid driver’s license, vehicle insurance, and access to a dependable vehicle or public transportation.
• CPR certification preferred or obtainable within 90 days of hire.
• 1+ years of clinical experience.

Responsibilities

• Make the initial nursing evaluation to determine hospice eligibility within 48 hours of referral.
• Identify patient/family needs and re-assess at least every 15 days.
• Initiate and coordinate the plan of care.
• Document problems, goals, interventions, and patient/family response to hospice care.
• Collaborate with patient/family, attending physician and IDG members in care delivery.
• Instruct and supervise patient/family in self-care techniques when appropriate.
• Maintain accurate clinical notes regarding patient condition.
• Provide care in home settings regardless of background.
