UnitedHealth Group, through Elite Hospice, is seeking a Registered Nurse Case Manager to oversee the Plan of Care for each hospice patient. You will collaborate with a multidisciplinary team to implement treatments that support physical, psychosocial, emotional, and spiritual well-being in home settings.

The role requires active RN licensure in the state and reliable transportation. The salary ranges from $71,644 to $86,944 annually, with full-time, benefits, and opportunities for growth within

Formación

• Current and unrestricted RN licensure in the state of practice.
• Current driver’s license or access to reliable transportation.
• CPR certification or ability to obtain within 90 days (preferred).

Responsabilidades

• Makes the initial nursing evaluation in determining eligibility for hospice services within 48 hours of referral.
• Identifies patient/family needs and reassesses at least every 15 days.
• Initiates and coordinates the plan of care.
• Documents problems, goals, interventions and patient/family response to care.
• Collaborates with patient/family, attending physician and IDG daily.
• Instructs and supervises self-care techniques when appropriate.
• Maintains accurate and relevant clinical notes.
