Job summary

Assured Hospice is seeking an RN Case Manager to oversee the Plan of Care for hospice patients in home settings. You will evaluate care, collaborate with the IDG, and implement treatment strategies focused on physical, psychosocial, emotional, and spiritual well-being.

Primary responsibilities include eligibility assessment within 48 hours of referral, care coordination, and patient/family education. Strong communication and adaptability to diverse home environments are essential.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current driver's license and access to a dependable vehicle or public transportation.
• CPR certification or ability to obtain within 90 days of hire.

Responsibilities

• Make the initial nursing evaluation to determine hospice eligibility within 48 hours of referral.
• Identify physical, psychosocial, and environmental needs; reassess at least every 15 days.
• Initiate and coordinate the plan of care and document goals, interventions, and responses.
• Collaborate with patient/family, attending physician and IDG for daily care.
• Instruct and supervise patient/family in self-care when appropriate; maintain accurate clinical notes.
