Job summary

UnitedHealth Group’s Northern Arizona Hospice is seeking a Registered Nurse Case Manager to lead the Plan of Care for hospice patients, collaborating with the care team to promote physical, psychosocial, and spiritual well-being.

Responsibilities include initial evaluation within 48 hours, developing and coordinating the plan of care, documenting progress, and ensuring compassionate, quality care across diverse home settings. Requires active RN licensure, valid driver's license, and CPR.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current driver's license and vehicle/transport access.
• CPR certification or ability to obtain within 90 days.

Responsibilities

• Perform initial nursing evaluation within 48 hours of referral.
• Develop and coordinate the plan of care with the care team.
• Document problems, goals, interventions, and responses.
• Collaborate with patient/family and IDG for daily care.
• Educate patient/family on self-care techniques when appropriate.
