Job summary

Luminary Hospice is seeking a PRN Registered Nurse Case Manager to plan and direct hospice care in patients' homes and communities. The role emphasizes nursing expertise, community health education, and coordinating services with families and a multi-disciplinary team.

The candidate should have nursing licensure, CPR, and a nursing degree; hospice experience is preferred. Responsibilities include assessments, care planning, medication administration, and guiding families in end-of-life care.

Qualifications

• Graduate of an accredited school of nursing.
• Current licensure in state and CPR certification.
• Bachelor’s degree; hospice care experience preferred.
• Management experience not required; may supervise aides.
• Strong observation, communication, problem-solving, and math abilities.

Responsibilities

• Performs comprehensive patient and family assessments to identify hospice needs.
• Develops and updates plans of care in collaboration with the team.
• Delivers nursing care in home/community settings using the nursing process.
• Coordinates care with physicians and interdisciplinary team members.
• Educates patients and families on self-care, medications, and plan of care.
• Maintains accurate clinical notes and documents care plans and outcomes.
