Job summary

Luminary Hospice is seeking a Registered Nurse Case Manager to plan, organize, and direct hospice care within patients’ homes and communities. The role emphasizes community health education and coordinating services to meet patient and family needs.

The ideal candidate will have a nursing degree, state licensure, CPR certification, and experience in hospice or home care. Responsibilities include performing assessments, creating care plans, coordinating with the care team, and supporting families

Responsibilities

• Completes initial and ongoing comprehensive patient/family assessments to determine hospice needs.
• Provides professional nursing care using the nursing process.
• Develops and revises care plans with patient/family involvement.
• Administers medications and treatments as prescribed and counsels patient/family.
• Maintains accurate clinical notes and communicates with physicians and care team.
• Acts as Case Manager coordinating patient care for assigned caseload.
• Participates in on-call duties and interdisciplinary group meetings.
