Qualifications

• One to two years of recent acute care experience in an institutional setting.
• One to two years of recent experience in home care.
• Current licensure in State and CPR certification.
• Responsible for supervising hospice aides.

Responsibilities

• Completes comprehensive assessments of the patient and family to determine hospice needs.
• Provides professional nursing care utilizing nursing process elements.
• Develops care plans that incorporate patient and family goals.
• Acts as Case Manager for assigned caseload.
