Job summary

Ecumen, a nonprofit senior living provider in Duluth, MN, is seeking a Hospice RN Case Manager to join our patient- and family-centered team. This full-time role delivers direct care and case management for home-based patients, with 64 hours bi-weekly and on-call coverage every 4th weekend and weekday shifts.

You will assess needs, develop and update care plans in collaboration with physicians and families, coordinate multidisciplinary care, and ensure compliance with agency policies.

Qualifications

• Associates degree in nursing and active RN licensure in the relevant state.
• Bachelor’s degree in nursing preferred.
• Strong communication skills, both verbal and written.
• Proficient with Microsoft Office Suite.

Responsibilities

• Delivers skilled nursing services and case management for home-based patients.
• Performs comprehensive assessments and develops a plan of care.
• Collaborates with physicians, other health care professionals, patients, and families.
• Updates physicians on patient status and changes to the care plan.
• Ensures physician orders are communicated to the care team accurately.
• Initiates case conferences to coordinate multidisciplinary care.
• Promotes safety for clients and coworkers.
