Job summary

Ecumen Hospice Twin Cities seeks a dedicated Hospice RN Case Manager to provide skilled nursing and case management in patients' homes and community settings. The role includes an 80-hour bi-weekly schedule, M-F 8am-4:30pm with occasional on-call, one weekend per month, and three holidays per year.

The RN will perform comprehensive assessments, develop and monitor end‑of‑life plans, coordinate with physicians and care teams, and ensure accurate communication of orders. Minnesota license required.

Qualifications

• Associate’s Degree in Nursing and RN license in Minnesota.
• Excellent written and verbal communication skills.
• Proficient with Microsoft Office Suite and safety policies.
• Knowledge of HIPAA and patient privacy requirements.

Responsibilities

• Delivers skilled nursing care and case management in home and community settings.
• Performs initial, comprehensive, ongoing patient and family assessments.
• Collaborates with physicians and care team to develop coordinated plans.
• Develops and updates end‑of‑life care plans and communicates changes to the team.
• Ensures orders are transmitted accurately to all care team members.
• Initiates case conferences and supports a safe environment for clients.
