AccentCare, Inc. is seeking a Hospice Registered Nurse (RN) to provide compassionate, patient-centered care in a hospice setting. You’ll work within an interdisciplinary team, building strong relationships with patients and families while coordinating care to ensure comfort and dignity during life’s delicate moments.

The role emphasizes autonomous practice at the top of your license, supported by a mission-driven organization committed to care for both patients and caregivers.

Qualifications

• Experience in hospice, home health, or palliative care preferred.
• Active RN license in good standing.

Responsabilités

• Provide individualized nursing care that maximizes comfort and quality of life
• Partner with patients, families, physicians, and the interdisciplinary group to create and carry out care plans
• Perform comprehensive assessments—physical, emotional, social, and spiritual—to guide care decisions
• Educate and support patients and caregivers, meeting them where they are
• Coordinate care across disciplines to ensure continuity and dignity
• Document care clearly and accurately to reflect patient needs and progress
