Job summary

Interim HealthCare is seeking a compassionate Hospice RN Case Manager to provide skilled nursing care in patients' homes, offering autonomy and meaningful one-on-one patient relationships. This salaried role features weekly bonus opportunities and a supportive team environment.

Qualified RNs will hold an active Michigan license (BSN not required), have hospice experience preferred, and possess CPR certification, reliable transportation, and current car insurance.

Qualifications

• RN with active, unrestricted license in Michigan; BSN not required.
• At least one year of clinical RN experience; hospice experience preferred.

Responsibilities

• Provide home-based care to patients with terminal illness.
• Work with a team of physicians and oversee CNAs providing patient care.
• Examine patients, perform assessments, document and report changes to physician.
• Educate patients on plan of care, medications, diet and exercise.
• Coach family members on care plan, medications and home safety.
• Coordinate discharge from services.
