Ecumen is seeking a Home Care Registered Nurse Case Manager in Duluth, MN to provide direct care and case management in home and community settings. The role supports client relationships with a focus on holistic care, safety, and coordination of services.

You will assess clients, collaborate with the team, and develop plans of care. The position requires an RN license and preferred Bachelor’s degree, with weekend and on-call rotations included.

Qualifications

• Associate's Degree in Nursing required.
• 1 year of nursing experience in post-acute care.
• RN license in state of practice.
• Strong communication skills (verbal and written).
• Proficient with Microsoft Office.
• Familiarity with safety and privacy policies.

Responsabilités

• Delivers skilled services efficiently and effectively to meet client and family needs.
• Performs initial, comprehensive, ongoing assessments of clients and families to determine needs.
• Collaborates with physicians and other health professionals to develop a coordinated plan of care.
• Develops a plan of care with objective goals for health improvement and evaluates progress.
• Ensures physician orders are communicated accurately to staff.
• Initiates case conferences to discuss multidisciplinary progress and care plans.
• Promotes personal safety and a safe environment for clients and coworkers.
