Ecumen is seeking a Hospice Registered Nurse Case Manager to provide direct patient care and case management in the North Branch, MN area. This full-time role features 80 hours every two weeks, with an on-call rotation including weekends, holidays, and after-hours.

The RN will conduct assessments, plan and coordinate end-of-life care, collaborate with the care team, and ensure accurate communication of physician orders. Local travel is required in home and community settings.

Formación

• Associate's degree in Nursing required.
• Must be a licensed RN in Minnesota.
• Excellent verbal and written communication skills.
• Proficient with Microsoft Office Suite and basic tech

Responsabilidades

• Delivers skilled nursing services to meet patient and family needs.
• Performs initial and ongoing patient assessments and histories.
• Collaborates with physicians and care team to create a comprehensive care plan.
• Develops goals for end-of-life care and updates plans as needed.
• Ensures orders are communicated accurately to care team.
• Initiates case conferences for multidisciplinary collaboration.
• Promotes safety for patients and staff.
