Job summary

Ecumen Hospice Twin Cities is hiring a Hospice RN Case Manager to provide direct care and case management in home and community settings. This role supports patients and families according to agency standards, with a schedule of Monday through Friday, 8:00 am to 4:30 pm, plus occasional on-call, one weekend per month, and three holidays per year.

Qualifications

  • Requires an Associate's Degree in Nursing.
  • Must be licensed as a Registered Nurse in the state of employment.
  • Ability to communicate effectively both verbally and in writing.
  • Proficient in Microsoft Office Suite.
  • Ability to follow safety rules, regulations, policies and procedures (HIPAA, Vulnerable Adult, etc.).

Responsibilities

  • Delivers skilled nursing services and case management to patients and families.
  • Performs initial and ongoing assessments of clients and families.
  • Collaborates with physicians and other health care professionals to develop a care plan.
  • Develops and evaluates end-of-life care plans and communicates changes to the care team.
  • Ensures physician orders are clearly communicated to the care team.
  • Initiates case conferences to discuss patient progress and plans for care.
  • Promotes safety and a safe environment for clients and staff.

Hospice RN Case Manager - Full-Time 80hrs bi-weekly - M-F 8am-4:30pm - Occasional on-call rotation - 3 Holidays per year and 1 weekend per month.

The Registered Nurse Case Manager position is responsible for providing direct care and case management for a team of patients in accordance with agency standards and policies. This position is responsible for performing skilled nursing functions in the home and community setting as well as serving the nursing needs of patients and families.

Schedule

Full-Time 80hrs bi-weekly M-F 8am-4:30pm occasional on-call requirements - 1 weekend per month and 3 holidays per year.

Pay

The targeted pay range for this job is $40.00-$45.00 per hour. Ecumen considers a variety of factors when determining new hire pay including experience, qualifications, and, when applicable, certifications, licenses, and education.

Essential Job Responsibilities
  1. 1. Delivers skilled services efficiently and effectively to meet patient's and family's needs.
  2. 2. Completes an initial, comprehensive, and ongoing assessment of clients and family to determine needs. Provides a complete physical assessment and history of current and previous illness.
  3. 3. Collaborates with physicians, other health care professionals, patients, and families in developing a comprehensive coordinated plan of care.
  4. 4. Develops plan of care which includes objective goals for patient's end of life care. Evaluates plan of care on continuous basis and updates physicians of changes in patient status and need for revision of plan of care.
  5. 5. Ensures that physician orders are communicated to all members of the care team with accuracy and thoroughness and understanding.
  6. 6. Initiates case conferences to discuss multidisciplinary team responsibilities, patient progress, plans for continued care, new problems, etc.
  7. 7. Promotes personal safety and a safe environment for clients and co-workers.

Minimum Required Qualifications
  • Associate's Degree in Nursing
  • Must be licensed as a Registered Nurse within the state in which working and must maintain licensure.
  • Ability to communicate effectively both verbal and written
  • Proficient with Microsoft Office Suite
  • Ability to follow all safety rules, regulations, policies and procedures of the facility including but not limited to: Vulnerable Adult and Abuse Policy, HIPAA, Resident's Bill of Rights, Universal Precaution, AWAIR, OSHA, Right to know, SDS, ADA, HUD Policy and Procedures, Fair Housing laws and regulations
Preferred Qualifications
  • Bachelor's degree in Nursing
  • Previous experience working with seniors
  • 1 year of nursing experience in post-acute care setting
