Job summary

LifeChoice Hospice and Palliative Care is seeking a compassionate Hospice RN to deliver patient-centered home hospice care. You will coordinate with an interdisciplinary group to meet physical, emotional, and spiritual needs while ensuring comfort and dignity for patients and families.

You will assess patients, develop personalized care plans, educate families, and document visits in compliance with federal and state standards.

Qualifications

• 1 year of RN experience; hospice experience preferred.
• Strong assessment and problem-solving skills.
• Excellent organizational and time-management abilities.
• Effective verbal and written communication skills.
• Commitment to patient dignity, safety and comfort.

Responsibilities

• Plan, coordinate, and deliver hospice care per individualized plans.
• Collaborate with patients, families, and IDG for seamless care.
• Conduct comprehensive assessments and develop care plans.
• Educate patients and families about end-of-life decisions.
• Document evaluations, visits, and care per standards.
• Supervise Hospice Aides and LPNs for quality service.
