LifeChoice Hospice and Palliative Care is seeking a dedicated Hospice RN to provide compassionate, patient-centered care at home and across care settings. You will coordinate with an interdisciplinary team to meet physical, emotional, and spiritual needs, ensuring dignity and comfort for patients and families.

The role emphasizes clinician leadership, ongoing education, and adherence to best practices in hospice care.

Formación

• RN license and nursing degree from an accredited school.
• Minimum of 1 year of RN experience; hospice experience preferred.
• Strong assessment and problem-solving skills.

Responsabilidades

• Plan, coordinate, and deliver hospice care according to individualized patient care plans.
• Collaborate with patients, families, and the interdisciplinary team to ensure seamless care delivery.
• Conduct comprehensive assessments and develop care plans tailored to each patient's needs.
• Provide education and guidance to patients and families about the dying process and healthcare decisions.
• Perform and document patient evaluations, visits, and care in compliance with standards.
• Monitor patient conditions and adjust care plans as needed.
• Supervise and evaluate care provided by Hospice Aides and LPNs.
