Job summary

Intermountain Health seeks a Hospice RN to deliver comfort-focused, patient-centered care in patients’ homes and community settings. You will use clinical judgment to assess symptoms, educate families, and coordinate services with an interdisciplinary team.

Qualifications

• Current RN license in state of practice.
• BLS certification for healthcare providers.
• Current driver's license, insured and reliable transportation, and an acceptable driving record.
• Ability to communicate effectively both verbally and in writing.
• BSN from an accredited institution. Degree will be verified.
• RNs with less than 12 months of RN experience must obtain BSN within four years of hire date.

Responsibilities

• Provide individualized, patient-centered hospice care in patients' homes.
• Coordinate services with an interdisciplinary team.
• Educate patients and families and provide emotional support.
• Assess symptoms and manage care using clinical judgment.
• Advocate for patients and families while working independently in home settings.
