Job summary

Lifecare Home Health is seeking an RN Case Manager to perform admissions and provide nursing care for patients in their homes across ages. You will coordinate care with the interdisciplinary team, patient families, and referral sources, ensuring timely and quality care in the patient’s residence.

The role requires two years RN experience in clinical settings and one year in hospice, with active RN licensure and a valid driver’s license.

Qualifications

• Two (2) years RN experience in a clinical care setting and one (1) year full-time hospice.
• Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
• Current State RN license; valid driver's license.

Responsibilities

• Assess hospice patients using Nursing Assessment and LCD criteria to identify needs.
• Develop and document the plan of care in correlation with the assessment.
• Provide nursing care with infection control measures (OSHA).
• Coordinate with Interdisciplinary Group/Team to meet patient/family needs.
• Communicate with attending physicians, hospice physicians, and other involved physicians.
• Timely submission of all required paperwork.
• Participate in hospice-sponsored in-service programs and QAPI.
• Meet productivity standards.
• Perform on-call duties for patients and families as assigned.
• Provide effective communication to patients, families, team, and other health professionals.
• Travel to patient homes as needed; transportation with valid liability insurance.
