Job summary

Angels Care Hospice in Omaha, NE is seeking a Hospice Registered Nurse (RN) for full-time home health care. You will coordinate patient care, perform admission visits, assess eligibility for home care, and develop the plan of care in collaboration with the care team.

Qualifications

• Graduate of an accredited nursing program (diploma/associate/baccalaureate).
• Current RN license in the state.
• Valid state Driver's License.
• One year of RN experience (two preferred).
• CPR certification and Hepatitis consent/declination.
• Reliable transportation with auto liability insurance.

Responsibilities

• Coordinate all aspects of the patient\'s home care with the care team.
• Perform admission visit to the patient at home and determine eligibility for home care.
• Develop and follow the plan of care for each patient.
