Job summary

Affinity Hospice Management LLC seeks an experienced registered nurse to plan, organize, and direct hospice care in patients' homes and communities, emphasizing education and comfort. This role involves comprehensive assessments, care planning, and coordinated nursing actions with the interdisciplinary team.

The position requires nursing licensure, strong communication skills, CPR certification, and a driver with insured vehicle.

Qualifications

• Graduate of an accredited school of nursing.
• Licensed RN in good standing.
• 1 year of professional nurse experience within last 3 years, or 2 years in public health/home care/hospice preferred.
• Management experience not required; may supervise hospice aides.
• Offer contingent upon references and criminal background checks.
• Excellent observation, verbal and written communication, problem solving, and basic math skills.
• Must have CPR certification and a valid driver’s license with insured vehicle.

Responsibilities

• Complete initial, comprehensive and ongoing assessments of patient and family to determine hospice needs.
• Provide professional nursing care using all elements of nursing process.
• Write and implement plan of care; regularly re-evaluate patient and family needs.
• Initiate and revise plan of care as patient status changes.
• Use health data to determine nursing diagnoses and goals.
• Develop care plans with patient/family; coordinate with physicians.
• Administer medications and treatments per physician’s plan.
• Counsel patient and family on nursing needs and self-care techniques.
• Provide health instructions and support continuity of care.
• Support on-call duties and ensure equipment and services are available.
