Angels Care Hospice is seeking a Registered Nurse (RN) Case Manager to coordinate all aspects of home care for patients in the community. You will perform admission visits in patients' homes and determine eligibility for services, then develop and manage the plan of care with the care team.

Qualifications

• Graduate of an accredited nursing program (diploma/associate/bachelor).
• Current state RN license.
• Current state Driver's License; reliable transportation.

Responsibilities

• Coordinate all aspects of the patient’s home care as RN Case Manager.
• Perform admission visits to patients in their home and assess eligibility for home care services.
• Develop and monitor the plan of care in collaboration with the care team.
