Job summary

Angels Care Hospice is seeking a Registered Nurse Case Manager to coordinate all aspects of in-home patient care and liaise with other medical disciplines. You will perform admissions, assess eligibility for home care services, and create comprehensive care plans tailored to each patient.

The role requires a nursing degree (Diploma/ASN/BSN), an active RN license, CPR, and reliable transportation for home visits. Nebraska-based, full-time with benefits and opportunities for professional growth.

Qualifications

• Graduate of an accredited diploma, Associate or Baccalaureate School of Nursing.
• Current state license as a Registered Nurse.
• Current state Driver's License.
• One year of RN experience; two years preferred.
• CPR certification and hepatitis consent/declination.
• Reliable transportation with valid auto liability insurance.

Responsibilities

• Coordinates all aspects of the patient's home care as RN Case Manager.
• Interacts with other medical disciplines as needed to provide professional care.
• Performs the admission visit in the home and determines eligibility for home care services.
• Develops and follows the plan of care for each patient.
