Job summary

Angels Care Home Health is hiring a Registered Nurse Case Manager to coordinate comprehensive home health care. You will visit patients at home, assess eligibility, and develop a care plan in collaboration with the care team.

Requirements include RN license in good standing, CPR, a valid driver’s license, and reliable transportation. A $5,000 sign-on bonus and strong benefits are offered to full-time nurses.

Qualifications

Responsibilities

• Coordinate home health care for patients as RN Case Manager
• Conduct admission visit to patient at home and assess eligibility
• Develop and monitor plan of care with interdisciplinary team
• Ensure documentation and compliance with regulations
