Job summary

Optum's Central Missouri Hospice seeks a dedicated Hospice Registered Nurse (RN) to manage the plan of care for each patient, collaborate with the IDG, and promote holistic well-being. The role includes 48-hour eligibility evaluation, regular reassessment, and family instruction in care techniques.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• 1+ years of clinical experience; hospice or home health preferred.
• Current CPR Certification.
• Valid driver's license, vehicle insurance, and access to a reliable vehicle.
• Ability to work with diverse patient populations and families.

Responsibilities

• Perform the initial nursing evaluation to determine hospice eligibility within 48 hours of referral.
• Identify patient/family needs and re-assess at least every 15 days.
• Initiate and coordinate the plan of care with the IDG and attending physician.
• Document problems, goals, interventions, and responses to care.
• Instruct and supervise patient/family in self-care techniques when appropriate.
