Job summary

Heartland Hospice in Missouri is seeking an experienced Registered Nurse Case Manager to lead compassionate end-of-life care at patients' homes or facilities. You will assess needs, develop plans of care, coordinate with an interdisciplinary team, and support families with education and bereavement resources.

You will perform ongoing in-person and telephonic assessments, ensure documentation quality, and advocate for patient comfort and dignity within regulatory guidelines.

Qualifications

• Current RN license valid for Missouri and CPR certification.
• Minimum 1 year RN experience in hospice, home health, or related field.
• Ability to work autonomously in patients' homes and coordinate care.

Responsibilities

• Perform comprehensive patient assessments and reassessments at home or in facilities.
• Deliver skilled nursing interventions and symptom management per Plan of Care.
• Develop and update individualized care plans with visit frequency.
• Coordinate care across IDG and consult with physicians and team members.
• Educate patients and families on disease, self-care, medications, and end-of-life options.
• Document thoroughly and maintain regulatory compliance.
