Job summary

Heartland Hospice seeks a dedicated Registered Nurse to serve as an RN Case Manager, delivering skilled nursing care at patients' homes or other settings. You will coordinate interdisciplinary care, educate families, and ensure compassionate end‑of‑life support.

The role requires strong clinical assessment skills, documentation discipline, and the ability to collaborate with physicians, social workers, and chaplains to meet individualized plans of care.

Qualifications

• Active RN license in the state of Illinois (or current authority to practice).
• Minimum 1 year RN experience in hospice, home health, or acute care settings preferred.

Responsibilities

• Be the primary contact for patients and families through the hospice journey.
• Provide and document skilled nursing care per the Plan of Care in the patient’s home or other setting.
• Perform initial and ongoing assessments and adjust care as needed.
