Job summary

Heartland Hospice is seeking a Registered Nurse Case Manager to lead in-home hospice care, coordinating with physicians, social workers and chaplains to deliver compassionate, patient-centered care. You will assess, plan, implement and document care, while guiding families through end-of-life decisions in patients' homes or alternative settings.

Ideal candidates have at least 1 year of RN experience in hospice or related fields, CPR certification, and a passion for education and bereavement

Qualifications

• RN license in state of employment.
• 1 year RN experience in acute care, hospice or home health preferred.
• Hospice/palliative care experience preferred.
• CPR certification.

Responsibilities

• Perform comprehensive patient assessments and reassessments.
• Deliver and document skilled nursing care per Plan of Care.
• Coordinate care with IDG and other clinicians.
• Educate patients and caregivers on disease, self-care and medications.
• Document clinical records timely and maintain infection control.
