Heartland Hospice is seeking a Hospice RN / RN Case Manager to deliver personalized end-of-life care in patients' homes or other living environments. You will assess, plan, implement, and coordinate nursing care with an interdisciplinary team, ensuring compassionate support to patients and their families.

The role emphasizes education, documentation, and adherence to regulatory standards, with a strong focus on quality, empathy, and timely communication across care teams.

Qualifikationen

• Active RN license in the state of employment.
• Minimum 1 year RN experience in hospice, home health, or related fields.
• CPR certification and driver’s license required.

Aufgaben

• Be primary contact for patients and families, guiding hospice journeys.
• Deliver and document skilled nursing care per Plan of Care.
• Perform initial and ongoing assessments and adjust care as needed.
• Collaborate with IDG to build personalized care plans.
• Provide emotional, physical, and spiritual support to patients and families.
• Educate caregivers and ensure accurate clinical documentation.
