Heartland Hospice is seeking a compassionate Registered Nurse to serve as an RN Case Manager, delivering skilled nursing care in the patient’s home or other living setting. You will assess patients, manage plans of care, and coordinate with physicians, social workers, and chaplains to ensure dignified end-of-life care.

You will educate families, document care accurately, and support emotional and spiritual needs while upholding infection control and regulatory standards.

Qualificações

• RN license in state of employment and CPR certification.
• Experience in hospice, palliative care, home health, or acute care preferred.
• Ability to document and communicate care plans and changes clearly.

Responsabilidades

• Perform comprehensive patient assessments and ongoing reassessments.
• Deliver and document skilled nursing interventions per Plan of Care.
• Coordinate care with physicians, social workers, chaplains, and other team members.
• Educate patients, families, and caregivers about disease, care plans, and end-of-life processes.
• Maintain accurate clinical records and comply with infection control standards.
