Job summary

Heartland Hospice is seeking a dedicated Registered Nurse Case Manager to provide expert hospice and palliative care in patients' homes and other settings. You will assess, plan, implement, and coordinate care with an interdisciplinary team to deliver compassionate, patient-centered services.

You will educate patients and families, ensure accurate documentation, and participate in on-call rotations, always upholding regulatory standards and quality care.

Qualifications

• Active RN license in the state of employment.
• Hospice or palliative care experience preferred.

Responsibilities

• Perform comprehensive patient assessments and reassessments in home or facility settings.
• Deliver skilled nursing care per individualized Plan of Care.
• Coordinate with IDG and physicians to develop and refine care plans.
• Educate patients, families, and caregivers on disease, end-of-life care, medications, and self-care.
• Maintain clinical documentation; ensure infection control and regulatory compliance.
• Participate in on-call and patient support activities as applicable.
