Job summary

Heartland Hospice is seeking an experienced RN Case Manager to lead in-home hospice care. You will perform comprehensive assessments, deliver skilled nursing, and coordinate with an interdisciplinary team to support patients and families in their homes or alternate settings.

The role emphasizes education, compassionate communication, and adherence to regulatory standards, with a strong focus on symptom management, care planning, and documentation.

Qualifications

• Registered Nurse with at least 1 year of RN experience in hospice/home health or related settings.
• CPR certification and current nursing license for the state of employment.
• Strong patient/family education and communication skills.

Responsibilities

• Perform comprehensive patient assessments and ongoing reassessments, educating patients, families, and caregivers as appropriate.
• Deliver skilled nursing interventions and symptom management aligned with goals of care.
• Develop and update individualized plans of care and visit schedules.
• Coordinate care with physicians, nurses, social workers, and chaplains to support patient needs.
• Document timely clinical records and ensure infection control and compliance standards.
