• Active RN license in Illinois
• Minimum 1 year RN experience in hospice, home health, or acute care
• CPR certification required

Responsibilities

• Be the primary contact for patients and families in hospice care
• Document Plan of Care and provide hands-on nursing in home settings
• Perform initial and ongoing patient assessments and adjust care as needed
• Collaborate with physicians, LPNs, CNAs, social workers, and chaplains to refine care plans
• Educate, comfort, and support patients and families, including bereavement guidance
