Job summary

Heartland Hospice is seeking a compassionate Registered Nurse Case Manager to lead patient care in homes and community settings in Missouri. You will perform initial and ongoing RN assessments, coordinate with an interdisciplinary team, and implement individualized plans of care with focus on comfort and quality of life.

You will educate patients and families, manage symptoms, maintain accurate documentation, and support end-of-life education and bereavement guidance.

Qualifications

• Active RN license in state of employment.
• CPR certification required.
• Current auto insurance and valid driver’s license.

Responsibilities

• Perform comprehensive patient assessments and ongoing RN assessments.
• Deliver skilled nursing interventions and symptom management.
• Develop and update individualized plans of care and visit frequency.
• Coordinate care with physicians, LPNs, CNAs, social workers, and chaplains.
• Educate patients and caregivers about disease, self-care, and end-of-life care.
• Document timely clinical records and ensure regulatory compliance.
