Job summary

Community Health Network is seeking a dedicated Hospice RN Case Manager to see patients in their homes in and around Dedham, MA.

This full-time role offers Monday–Friday, 40 hours per week, with no weekends or on-call required and competitive pay with mileage reimbursement.

Qualifications

• Requires minimum 1 year of Hospice RN experience with a Medicare certified agency.
• Direct patient care, coordination of patient and patient advocacy.

Patient includes the individual, family/significant other and community. Must be comfortable with issues relative to end-of life care, death and dying.
