Hospice RN Case Manager

Employment Details

Full-time, Monday-Friday, 40 hours/week. Shorter work weeks considered. No weekends or on-call required. Great pay rate plus mileage reimbursement.

Location

Patients in group homes and facilities in Tewksbury/Reading/Danvers/Peabody, M?A and surrounding area.

Responsibilities

Primary nurse provides quality nursing care to identified Hospice patients. Scope of the role includes direct patient care, coordination of care, and patient advocacy. The patient includes the individual, family/significant other, and the community. Must be comfortable with end‑of‑life care issues, death, and dying.

Requirements
