Job summary

AdventHealth is seeking a dedicated Registered Nurse to deliver hospice nursing services in home or facility settings across Central Florida. You will coordinate and implement the Plan of Care for an assigned caseload, monitor patient conditions, and educate families on disease processes and safety.

You will partner with physicians and the interdisciplinary team to adjust care plans, order resources, and communicate changes in medications.

Qualifications

• RN with current Florida license (or permitted/compact state).
• 3+ years home health and/or acute care nursing experience preferred.
• Ability to educate patients, families, and caregivers.
• Knowledge of InterQual criteria and care planning.

Responsibilities

• Provides hospice nursing services in home or facilities.
• Manages plans of care for assigned caseload.
• Monitors patient conditions and conducts regular assessments.
• Collaborates with physicians and a care team to adjust plans.
• Orders resources and communicates medication changes.
