Job summary

The position requires an RN license, BLS certification, and experience in home health or acute care settings. Documentation is done in the EMR system, and collaboration with caregivers is essential.

Qualifications

• Associate's degree in Nursing required; Bachelor's degree preferred.
• Registered Nurse license required; Basic Life Support (BLS) certification required.
• Home health and/or acute care nursing experience preferred.

Responsibilities

• Provide hospice nursing services at home or in facilities.
• Complete required documentation in the EMR system.
• Support family members and caregivers with care plans.
• Participate in on-call rotations for after-hours coverage.
