Job summary

AdventHealth Hospice Care in Florida is seeking an experienced Registered Nurse to assess patients for hospice eligibility, coordinate referrals and support families. Travel to Orange, Osceola and Seminole Counties is required.

You will document in the EMR, obtain orders, and collaborate with the Hospice team to ensure timely admissions and quality care. Schedule options include hospital shifts or field-based coverage with weekend rotations.

Qualifications

3+ years RN experience in home health and/or acute care, including 1-year hospital experience.
RN license in Florida with CPR/BLS.
Ability to document accurately in electronic systems.

Responsibilities

Travels to patients' locations to assess hospice eligibility and discuss services.
Collaborates with Account Representatives to meet referral needs.
Obtains orders and completes documentation for reimbursement.
Evaluates patients' medical needs and equipment requirements.
Notifies Hospice team for crises and coordinates admissions.
Documents patient information in the EMR promptly and accurately.
Arranges patient transportation based on circumstances.
