Zusammenfassung

Advocate Health is seeking a Hospice Registered Nurse to provide comprehensive end-of-life care, education, and care coordination for patients and families in hospice settings. The role requires 1–2 years of nursing experience and an RN license, with on-call rotation and a 40-hour work week.

You will educate families, lead the IDG in developing care plans, and document thoroughly in the electronic health record while ensuring safety and quality of life for patients and their loved ones.

Qualifikationen

• Education: Completion of an accredited or approved program in Nursing.
• Experience: Typically requires 1–2 years of nursing experience, including home health or hospice.
• Licensure/Certification: Registered Nurse license required; Basic Life Support (BLS) for Healthcare Providers certification may be required.

Aufgaben

• Identify and coordinate patient/family care to support terminally ill patients and families in hospice settings.
• Educate patients and families about disease processes, hospice care, and end-of-life signs.
• Review caseload to ensure appropriate level of care and refer to team members as needed.
• Lead IDG meetings to develop and update the Plan of Care for each patient.
• Document in the Electronic Health Record per guidelines and ensure accurate patient records.
• Supervise Home Health Aide and LPN visits and mentor staff as requested.
• Facilitate discharge planning and coordinate community resources for continuity of care.
• Enter orders and communicate changes in patient condition to physicians and IDG.
