Job summary

Chapters Health System is seeking an experienced RN Case Manager to provide compassionate, in-home hospice and palliative care under a multidisciplinary team in Florida.

The role involves coordinating the Plan of Care, visiting patients at home, and educating families while helping patients remain in their homes as long as possible. Monday–Friday 8am–5pm; no on-call or weekends; sign-on bonus and benefits begin day one.

Qualifications

• Current RN license in Florida.
• Minimum 1 year nursing experience in hospice or hospital setting.
• Experience with EMR/EHR systems.
• Valid driver’s license and reliable transportation.
• Ability to communicate effectively in English (verbal and written).
• Ability to visit participants in their homes for assessments.

Responsibilities

• Assess patient/family needs and coordinate Plan of Care with IDT.
• Provide clinical, palliative and supportive care.
• Conduct initial and semi-annual assessments for company services.
• Communicate with physicians and IDT using SBAR.
• Document patient/family information in EMR and share with IDT.
• Meet home safety and care needs during visits and follow care protocols.
