Medlinkhealth Anesthesia Associates is seeking an RN Care Manager for hospice services in Roanoke, VA. The role focuses on compassionate, high-quality nursing care to hospice patients and their families in home settings, with travel within the service area.

The ideal candidate holds an accredited nursing degree, an active Virginia RN license, and at least 2 years of related experience. BLS certification is required, and mileage reimbursement is offered.

Qualifications

• Graduate of an accredited nursing school.
• Active Virginia RN license, verified through Nursys.
• 2+ years of experience in acute care, home health, hospice, medical-surgical, or geriatric nursing.
• Current Virginia driver's license.
• Driving record must meet policy standards.
• BLS (AHA) certification.

Responsabilités

• Manage patient care and provide specialized nursing services to hospice patients.
• Assess and manage patient symptoms and respond to care-related crises.
• Educate patients and families on care plans and symptom management.
• Develop and maintain positive relationships with patients and families.
• Collaborate with the interdisciplinary healthcare team to ensure quality patient care.
• Maintain effective written and verbal communication and demonstrate strong problem-solving and decision-making skills.
• Provide care primarily in patients’ homes, with travel to long-term care facilities, hospitals, and other locations within the service area.
