Qualifications

• Must be a Registered Nurse with a current nursing license in Virginia.
• At least one year of home health experience preferred.
• Must have a valid driver's license and a reliable car.

Responsibilities

• Provides skilled nursing care to homebound patients per the established Plan of Care.
• Coordinates and manages multidisciplinary services to ensure continuity of care.
• Educates patients and families on health maintenance and disease prevention.
